Job Summary :

The Physical Therapist provides home health physical therapy services to patients in Salem OR and surrounding areas. This role involves assessing patient needs developing and implementing treatment plans and working to improve patients mobility and overall well-being. The therapist will manage a caseload of patients ensuring appropriate documentation and communication with the healthcare team.

Location : Mount Angel Oregon United States

Responsibilities :

• Conduct patient assessments to determine physical therapy needs.
• Develop and implement individualized treatment plans.
• Provide skilled physical therapy interventions to improve mobility and function.
• Document patient progress and communicate with the healthcare team.
• Manage a caseload of home health patients optimizing visit schedules.
• Ensure accurate and timely completion of Oasis documentation.
• Maintain compliance with facility policies and procedures.
