job_description.job_card.job_descriptionPerform primary care in a post-acute setting for short term and long-term patients Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care Order and interpret lab results Prescribe and monitor medications Consult with physicians and coordinate patient care with other healthcare practitioners Educate patients and their caregivers on coping skills to handle chronic and age-related conditions Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery Promote healthy lifestyle changes A minimum of 1-2 years' experience as a nurse practitioner or physician assistant in a primary care setting required. Will accept new grads Post-acute care experience a plus, but not required Active and unrestricted New York NP or PA license required Active and unrestricted DEA with prescriptive authority required

Job Details
- 15 patients per day
- Full-time position : 4-5 days per week, Monday through Friday, day call weekends
- Compensation and benefits : our compensation is fee for service (FFS); expected compensation is estimated salary range of $132,184 - $156,621 annually with no cap on productivity income potential. This full-time role is benefit eligible
