The Comprehensive Assisted Recovery & Empowerment (CARE) Program Case Manager will play a critical role in managing patient care, improving outcomes, and coordinating with various members of the healthcare team. The Case Manager will work with the healthcare team and community partners to develop and implement treatment plans that incorporate both medical and behavioral health interventions to ensure that patients receive the appropriate level of care based on their individual needs.
The CARE Program utilizes a team-based collaborative care model to support patients with co-morbid mental health disorders and substance use disorders or chronic pain in the primary care setting. The goal of the CARE Program is to improve both mental and physical health. Through intensive case management, the Case Manager contributes to improving patient outcomes and enhancing the overall quality of care provided by the Forest Home Health Center Family Medicine clinic.
The ideal candidate for this position will be a highly motivated and compassionate healthcare professional with a passion for improving patient care outcomes. Support caseload of patients enrolled in Comprehensive Assisted Recovery & Empowerment Program for collaborative care case management, working closely with the healthcare team to ensure seamless communication and coordination between team members.

Position Requirements :
Minimum Qualifications :
Appropriate experience may be substituted for education on an equivalent basis.
Minimum education : Bachelor's degree in social work
Minimum experience : 3 years
Certifications : Social Work license
Preferred Qualifications :
Preferred education : Master's degree
Preferred experience : 3 years of experience working with patients who have substance use disorders and mental illness. Strong communication and interpersonal skills. Working knowledge of Epic Compass Rose is a plus; willingness to learn is acceptable-training will be provided.
