job_description.job_card.job_descriptionMust be registered with the BBS as an ACSW, AMFT, APCC or be registered with the BBS as a Licensed Clinical Social Worker in the State of CA. A minimum of two years’ experience providing outreach or case management support to persons experiencing homelessness, or at risk of homelessness, including working with persons experiencing chronic health issues, mental health needs, or with histories of substance use. At least one year of supervisory experience in a social service organization is preferred. Bi-lingual English / Spanish preferred. Work indoors in a temperature-controlled environment with exposure to outdoor weather and driving conditions several times a week. The noise level in the work environment is usually moderate. Stand and sit for long periods of time; move and walk to various locations; climb stairs, stoop, kneel, reach, pull, push, bend, twist, and occasionally lift and or move objects. Use hands to finger, handle or feel. See, hear and speak clearly in order to give and receive information and instructions, perform administrative work, and drive motor vehicles including passenger vans. Ability to interact with other employees, clients, customers and members of the public. Ability to work effectively under time constraints, prioritize work, multi-task, and adapt to changing work demands. Gather, analyze, synthesize, and classify information. Transcribe, enter, and post data. Ability to respond effectively to sensitive inquiries or complaints. May require exposure to blood borne pathogens and infectious agents. Requires use of a computer keyboard for substantial periods of time. Able to travel to off-site meetings or other activities. Must be highly motivated and a self-starter. The ability to communicate with and relate to a diverse group of people including clients, community, and other staff. Must have excellent organizational skills and the capability to work in a fast paced environment. Knowledge of crisis and permanent housing resources throughout Los Angeles County. Knowledge of housing issues in San Gabriel Valley preferred. Knowledge of HUD guidelines and experience working with subsidized housing programs. Current knowledge of best practice models, including : Housing First, Trauma Informed Care, Mental Health Recovery, Harm Reduction, and Critical Time Intervention. Proficient in use of laptop computer and Microsoft Windows, Office software programs, and Google Apps. (Word, Excel, PowerPoint, G-mail, Docs). Ability to accurately enter client data and progress notes in the Homeless Management Information System, CHAMP, and other databases as needed and run program management reports. Proficient with office communication equipment and machines (telephone, copier, calculator, fax machine, personal computer, laptop, printer and scanner). Understanding of case management practices and ability to accurately and clearly document case notes. Excellent written and verbal communication skills. Possess valid California driver's license and have access to properly registered vehicle.

JOB SUMMARY :
The Clinical Program Manager for the PORT Team will provide oversight of day to day outreach activities through supervision of the Peer Outreach Specialists on the PORT Team and collaboration with the Pasadena Public Health Department. The Clinical Program Manager manages all administrative and program reporting, tracks program outcomes and ensures accuracy and timeliness of program documentation. The Clinical Program Manager is also responsible for providing clinical services on the team, including field-based mental health and substance use assessments, short-term and crisis interventions, client care plans, clinical consultations, and resource linkage.
ESSENTIAL FUNCTIONS INCLUDE , but are not limited to the following :
- Provide direct supervision, training, and support for Peer Outreach Specialists, including hiring, leading staff meetings / case conferencing, and conducting performance evaluations for program staff.
- Collaborate with the Pasadena Public Health Department to support the PORT team in meeting performance standards and working toward improved service delivery that is trauma-informed and aligns with Housing First and Harm Reduction.
- Provide clinical services through regular co-location and field-based outreach activities, including mental health and substance use assessments, short-term and crisis interventions, client care plans, clinical consultations, and resource linkage.
- Build trust and rapport with individuals experiencing homelessness, community partners, and other staff.
- Ensure that services provided to outreach participants are voluntary, person-centered, and client-driven.
- Participate in Prop 47 Advisory Committee and PORT evaluation activities.
- In coordination with the Data & Quality Assurance team, review case files and QA processes to ensure compliance with reporting and documentation guidelines, in preparation for audits.
- Monitor and approve program expenditures, including petty cash, debit card expenditures, and check requests, to ensure the effective management of program budgets.
- Track and provide statistical data to support grant requirements.
- If applicable, provide supervision to MSW interns working within the program.
- Other duties as assigned.
